In order for your child to receive this vaccination when we visit your school on Friday 14th November 2025, please complete the e-consent form by Monday 10th November 2025.

• Please visit www.surreyimmunisations.co.uk and enter your school’s unique code SR125294

• If you do not want your child to receive a vaccine, we would ask you to still complete a consent form but select the ‘No’ option.

• If you do not want your child to receive the nasal spray vaccine on the basis of its porcine gelatine content, please fill in the ‘No’ box indicating this, and you will be given the opportunity to consent to an alternative vaccination. This vaccination is delivered by injection into the arm and does not contain any porcine gelatine. We will deliver this in school when we visit. If, however, you would like your child to receive this vaccination in one of our clinics, please contact us on the above number or email.

An automated email will be sent to you on completion of the e-consent from school.immunisations@nhs.net. If you do not receive this, please check your junk email.

Measles, Mumps, Rubella (MMR)

By now your child should have had two doses of MMR to fully protect them. If your child has NOT had two doses of MMR, and you would like them to be protected, please complete the MMR consent form. Find out more about this vaccine at: MMR for all: general guide
